TLP is a supervised, community based, scattered-site apartment program, which provides an opportunity for young people to live independently with necessary supervision and support. ILP is not a direct provider of housing; instead this program is administered by licensed Child Caring Institutions and works in partnership with existing providers.
GOALS
- To encourage youth responsibility while transitioning youth from agency dependence to self sufficiency
- To provide affordable, safe and stable housing for youth in transition
- To provide vocational and training opportunities that will generate job security and resources sufficient to meet daily needs
- To provide personal and emotional support via a Life Coaches/Mentor, which will also promote the interaction with dedicated supportive adults
- To encourage youth to assume greater responsibility for their success
ELIGIBILITY
- Young adults between the ages of 18 and 21
- Signed Extended Supportive Services (formerly the "Remain in Care" form)
- Employed
- Adolescent Assessment - Comprehensive Child & Family Assessment (CCFA)
- Written Transition Living Plan (WTLP)
- Life Skills Training (employment retention, money management, household and health maintenance)
START UP COSTS
ILP provides direct support for youth who have been accepted into a TLP for the following items below:
- Security deposits
- First month's rent
- Utility connection fees
- Basic furniture (bed, chest of drawers, table and chairs)
- Minimum deposits to open savings and checking accounts
- Funds necessary to secure state identification
- Toiletries (sheets, towels, bed linens, pots pans, dishes and other kitchen utensils, iron and alarm clock)
